Doctor Visit Checklist: What to Bring and Ask
A plain, printable checklist for any doctor visit — checkup, specialist, or follow-up. Use it on paper, or let Health Visit App keep it for you automatically. No signup required to use this page.
Before you go
- Photo ID and insurance card
- Current medication list — names, doses, and how often you take each one App: medication vault
- A short note on the main reason for this visit, in your own words
- Any symptoms — when they started, what makes them better or worse
- Recent test results, labs, or imaging you already have copies of
- Names and contact info for your other doctors, if this is a specialist visit
- Payment method for a copay, if you have one
Questions to bring with you
Pick your top 2–3. A short list you actually ask beats a long list you run out of time for.
- What is this diagnosis or finding, in plain language?
- What are the options, and what would you recommend?
- If a new medication is prescribed: what's it for, and what side effects should I watch for?
- When and how will I get results, if tests are ordered?
- What should I do if things get worse before the next visit?
- When should I schedule a follow-up? App: Visit Prep AI Interview
During the visit
- Ask your top question early, so it doesn't get skipped if time runs short
- Take notes, or bring someone who can
- If you want to record the conversation, ask first — every time, and only with a clear yes
- Repeat back what you heard, to confirm you understood it correctly
- Ask for written or printed instructions if any are available
This checklist is for preparation and organization. It isn't medical advice, and it doesn't replace
your clinician's judgment. For anything urgent, call 911 or your local emergency number.
After the visit
- Write down what you were told while it's fresh — don't rely on memory alone App: After-Visit Checklist
- Note any follow-up date, and put it on your calendar
- Update your medication list if anything changed
- Request copies of new test results or records for your files
- Fill any new prescriptions, and note refill dates
